About the Role:
Working as a Community nurse, as part of a team of nurses, the post holder will be responsible for the delivery of high-quality care to patients in their own homes or community settings, through the management of a caseload of patients within a community setting.
The post holder will work collaboratively with other health care professionals within primary care, trust multidisciplinary teams, social workers, GP’s and individuals, to assess, plan, implement and evaluate patient care, focusing on health promotion and disease prevention.
The role demands excellent clinical knowledge, effective communication skills and sound community awareness. The HCN will have the ability to work independently and as part of the community. As a team member the HCN will contribute to a positive supporting working environment with an emphasis on teamwork and co-operation.
Person Specification
Essential Criteria
- Registered Nurse on the NMC Register Adult level 1.
- Has 1 years’ experience, post registration experience (or part time equivalent) working in a hospital or community setting
- Must hold a full, clean UK driving licence and have access to a reliable car for community-based work.
- Compassionate, empathetic, and patient-focused approach to care.
- Strong work ethic, attention to detail, and commitment to high standards
- Excellent communication skills, both written and verbal, with the ability to engage and collaborate with patients, families, and multidisciplinary teams
- Strong knowledge of appropriate record keeping in line with RQIA and NMC standards
- Experience in problem solving in a care setting, to be able to analyse complex situations and make appropriate judgments
- Experience in managing a caseload of patients and developing individual care plans.
- Strong clinical skills, with the ability to assess patient needs, provide hands-on nursing care, and respond to medical emergencies effectively.
- Ability to identify and address challenges, with practical solutions that ensure patient safety and well-being.
- Ability to motivate and lead a team, support colleagues, and guide new staff during their induction.
- Flexibility and the ability to adapt to changing circumstances, managing different patient needs, and working within varying community settings.
- Able to work independently and as part of a team, with a collaborative mindset.
Desirable Criteria
- Experience working in community health or homecare settings.
- Actively engaging in post-registration training in areas such as wound care and palliative care.
- Familiarity with health and social care policies specific to the northern trust or sector.
- Proficiency in using electronic care management systems and basic office software.
- Proactive attitude towards professional development and continuous learning
